The Case Management and Coordination team provides case work support to a range of individuals, families and carers to support aspirations, growth and change within a strengths based approach. This may include support and advocacy through the systems which significantly impact on individual and family lives, such as; child protection; income support; housing; mental health and healthcare; disability; education and legal, parenting and early years.
This position involves both centre-based and outreach functions involving coordinated work with a range of agency, community and staff resources. Kyabra works in an integrated model allowing case workers to apply their skills and knowledge to a range of community needs.
This case worker position will work across the areas of foster and kinship care and NDIS support coordination. Our foster and kinship care service provides a high standard of case management to both general and kinship carers and the children in their care. Our NDIS supports coordination services adopts a person centred approach to assisting individuals and their nominees to navigate the NDIS and implement a range of supports and services that facilitates the achievement of individualised plan goals. We do this through the use of various strengths-based tools and resources.
